What is a Privacy Notice?

Under data protection law you, as client of Wedmore Physiotherapy, you have specific rights. To communicate these rights to you in a clear and concise manner, we are providing you with this privacy notice.

Who I am

I am Karen Larsen, Wedmore Physiotherapy, The Surgery, St Medard Road, Wedmore, BS28 4AN. Telephone number 07515532808, email address wedmorephysio@gmail.com. For the purposes of processing your personal data I am the Controller.

Data Protection Officer

As I record and use sensitive data I take the protection of this data very seriously. I have therefore reviewed and updated all my processes for data collection and storage. Please contact me for any matters regarding your personal data I process.

The Personal Data I Process and What I Do with It

I record and use the following categories of personal data which may include: name, address, telephone numbers, email address, as well as date of birth, and physiotherapy and health records.

Sharing Your Personal Data

I only share your personal data with your explicit consent, where, for example I need to contact your general practitioner or consultant with regards to your treatment, or your medical insurer, again with regards to treatment . Where third parties are used by me to store your personal data, I have ensured they are compliant with the General Data Protection Regulation.

Retaining Your Personal Data

Physiotherapy records are saved for eight years for adults, after conclusion of treatment. For children and young adults, the records are kept until the client has reached the age of 25 or for eight years after completed treatment. Contact details will be saved for the same duration.

Your Rights

As I process your personal data, you have certain rights. These are a right of access, a right of rectification, and a right to restrict processing.

Data Breaches

Should your personal data that I control be lost, stolen or otherwise breached, where this constitutes a high risk to your rights and freedoms, I will contact you without delay. I will explain to you the nature of the breach and the steps we are taking to deal with it.

You can contact the ICO via their website:www.ico.org.uk should you wish to make a complaint about the way I am processing your personal data.

Automated Decision Making and Profiling

I do not use any system which uses automated decision making or profiling in respect of your personal data.
